REGISTRATION MAILING CHECKLIST Must be postmarked 1/16/10 to avoid late fees

Printed registration invoice with head coach and supervisor signature
Additional registration print outs for All-State and Stunt Groups
Entry fee check/money order made payable to WACPC, Inc.
Printed online apparel pre-order invoice and payment for Regionals, All Star, and JAM competitions
School's "Certification of Exempt Status" (CES) form or number to avoid paying tax on your ordered merchandise
Completed facility agreement form
Completed medical release forms for each participant
Submit all materials to:  WACPC, Inc. 1408 S. Covenant Lane Appleton, WI 54915

COMPLETE RULES QUIZ Submit no later than 1/22/10 11:59pm CST

Head coach is required to take online rules quiz and pass with a score of 85% or higher prior to competing at WACPC events
Programs with multiple teams competing should have the head coach at each level complete the quiz
Utilize the NFHS rules book provided with your membership in preparation for this quiz

TEAM AND INDIVIDUAL PHOTOS DEADLINE HAS BEEN EXTENDED UNTIL 1/18/10 FOR VARSITY TEAMS

Photos should be emailed to photos@wacpc.com to be included in state programs
Make sure to include the following information in the title line of your email:  Team/Individual Name and Sport (Cheer, Dance, All Star)
Photos should be sent as an attachment in JPG format
Photographers, assistant coaches, athletes, or parents may submit photos, but make sure to include head coach's name with correspondence
Team photos must have a HORIZONTAL orientation
Individual All-State and Coach of the Year candidate photos must have a VERTICAL orientation; head shots are best
